Appointment Policy

Our providers see patients by scheduled appointments. As a courtesy to other patients in the practice, we ask that if you need to cancel an appointment, do so as early as possible.

Since we realize emergencies happen, please make sure to alert the scheduler when calling as to the reason you are canceling, especially if it is less than 24 hours before the appointment. 

If you simply need lab work or an injection you already have orders for, you may walk-in from 7:30 to 11:30 and 1:30 to 4:30 to have that done by one of our nurses or lab technicians.

Our clinic is open on Saturdays starting at 8 am for sick appointments only. Patients are seen on a first-come, first-served basis. No appointment is necessary. Call Family Practice Associates today to learn more.

No-Show Policy

Should you miss two or more appointments without any prior notice or less than 24 hours notice, you might be discharged from the practice. Patients who are new to our clinic and miss their first appointment with our office without calling ahead of time to cancel may not be allowed to schedule with our clinic again.

Please note we charge a $25 fee for not showing up for a scheduled appointment or canceling the same day with one of our physicians.

Self-Pay Patient Policy

We understand that sometimes those without insurance delay visiting the doctor for fear of costs. We would like to dispel that fear as much as possible.

Most of our office visits range from $60 to $100. We ask that current self-pay patients be able to pay at least a $56 deposit toward the office visit on the day of the appointment, and the remainder can be paid within 60 days.

Insurance Policy

Our billing department is readily available to answer your insurance questions before, during, and after your appointment. Since there are so many ever-changing plans, we encourage you to review your plan information and call your insurance for any specific coverage inquiries.


We will be more than happy to supply specific coding information if you would like to ask your insurance company about coverage for a certain procedure. Please be aware that applicable co-pays, co-insurance, and deductibles are due at the time of service.

Medicare (most replacement plans as well, EXCEPT Humana)

Please understand we will file your insurance as a courtesy to you, but if your insurance does not pay within a reasonable amount of time, the bill will be your responsibility to pay in full. Call us to learn more.

Discharged Patient Policy

Please be aware that patients who are discharged from our clinic for any reason (non-compliance with doctor's orders, excessively missed appointments, non-payment of a bill, and more) will also be responsible for the discharge of their immediate family from our clinic, any family members directly related to the patient and living in the same household including but not limited to husband, wife, son, daughter, mother, and father.
